We require all new clients to fill out our clinic forms. These forms are what convey all the information to your therapist for them to get a picture of your symptoms and your needs. We rely on them to remind us of where your journey began, so that we can measure your progress over time.
We understand the importance of your personal information and keep everything you tell us, both on our forms and in your appointments, strictly confidential. Your e-mail address is only for our own use in communicating with you, as is your mailing address and telephone number. Your credit card information is kept confidentially as well. We do not reveal any information to anyone outside of the clinic, even your physician, without your permission first.

Client Intake Form
If this is your first appointment for any therapy, you will need to fill out and bring this form with you to your appointment. The first page is yours to keep because it details our policies for you. There is more policy information on our website under Clinic Policies.
Digestive Intake Form
If this is your first appointment for Colonic Hydrotherapy or a Cleanse Consult, you will need to fill out and bring this form with you.
Skin Care Form
If you are receiving Lymphatic Drainage or our ISIS Botanical Facial for the first time, then please print out this form, fill it out and bring it to your appointment.
Naturopathic Evaluation Client Form
If you have not had a Naturopathic Evaluation within the last two years, please print this form, fill it out completely and bring it with you to your first appointment.
Detox Battle Plan Form 1
You will need this form along with our Toxicity Questionnaire for our Detox Battle Plan appointment. Please download this form, fill it out and bring it with you to your appointment.

ChiroWellness Client Form
Please fill out this form if you are receiving your first ChiroWellness appointment at ISIS. If you are a new client to ISIS clinic, then you will also need to fill out the Client Intake form as well.
ChiroWellness Form for Minor Child
If the patient receiving a ChiroWellness appointment is a minor child under 18, then this form must be filled out by the parent or guardian. Also the ChiroWellness Client form must be filled out in addition to this form. If the minor child is coming to ISIS for the first time, then also add the ISIS Client form.
